Community Access Program

Helping make midwifery care and out-of-hospital birth more accessible to families in our community.

At Birth & Wellness Center of Arlington, we believe families should have access to respectful, personalized maternity care regardless of their financial circumstances. Our Community Access Program (CAP) offers adjusted pricing and alternative payment arrangements for eligible families who may otherwise find the cost of care out of reach.

You may qualify for the Community Access Program based on:

  • Your household size and income
  • Participation in a qualifying assistance program, such as Medicaid
  • A documented financial hardship or significant change in circumstances

Eligibility is determined individually based on the information and documentation provided. Because every family’s situation is different, CAP adjustments and payment arrangements may vary.

How to Request Community Access

  • Contact our office and request a Community Access Program Request Form.
  • Complete the form and provide the requested income or eligibility documentation.
  • Our administrative team will review your information and determine whether you qualify.
  • If approved, your CAP adjustment will be included in your personalized Good Faith Estimate and Financial Planning Summary before you begin care.

Participation in the Community Access Program does not change the quality, attention, or services you receive. It simply allows us to structure the financial portion of your care in a way that better reflects your family’s circumstances.

Please note: The Community Access Program applies specifically to eligible services provided by Birth & Wellness Center of Arlington and Leslie Jackson, CNM. It does not apply to services provided by other independent practices, laboratories, sonography providers, or outside healthcare professionals.
